Registered Nurse – Case Manager (Day Shift)

Position Summary

Henry Ford Jackson Hospital is seeking experienced RN Case Managers to support interdisciplinary teams in managing patient care across the continuum. This role focuses on discharge planning, utilization management, and care coordination to improve clinical outcomes and patient satisfaction while managing cost and ensuring timely reimbursement.

Key Responsibilities

• Facilitate precertification and payor authorization
• Coordinate collaborative patient care and remove barriers to efficient delivery
• Support clinical documentation and coding accuracy
• Serve as a resource for physicians, coders, and patients
• Apply process improvement methodologies to evaluate care outcomes
  
  

Key Requirements

• Experience:
• Minimum 2 years of case management experience (utilization-only experience not accepted)
• Education:
• Required: Bachelor of Science in Nursing (BSN)
• Certifications:
• Required: Active Michigan RN license
• Preferred Experience:
• Familiarity with Care Manager and Epic systems
• Discharge planning experience
• Travel Experience: Not required
